ACP does not support “Comprehensive Diabetes Care: Blood Pressure Control (<140/90 mm Hg)” at the health plan level because of uncertain validity. The measure aims to improve the quality of care for patients with diabetes by assessing whether their blood pressure was adequately controlled (<140/90 mm Hg). High blood pressure is a significant risk factor for microvascular and macrovascular complications in patients with diabetes. An individualized assessment to achieve a specific target is the appropriate approach to account for benefits and harms. There are concerns with the numerator only counting the most recent blood pressure reading; this negates any progress the patient may have made throughout the year. To make the measure more meaningful, the measure developers need to alter that component by allowing the use of the average BP during the measurement period. Using the most recent blood pressure reading is not as accurate and does not necessarily improve the patient’s cardiovascular health.
