Measure Info
ACP supports “Hemoglobin A1c (HbA1c) Control (<8.0%)” for application at the health plan level. The measure accounts for variability in patient populations with a less stringent glycemic goal. ACP’s guidance statement on HbA1c targets for glycemic control recommends that physicians personalize goals for glycemic control in patients with type 2 diabetes. Glycemic control goals require a discussion of the benefits and harms of pharmacotherapy, patients’ preferences, patients’ general health and life expectancy, treatment burden, and costs of care. While the outcome of this performance measure is out of the physician’s control and relies heavily on patient adherence, the data is easy to collect.
