ACP supports “Hemoglobin A1c (HbA1c) Poor Control (>9%)” at the health plan level. Each percentage drop in A1c levels leads to a lower risk of complications and better patient outcomes. Physicians should encourage patients with diabetes to test their A1c levels twice a year to maintain a proper A1c level. ACP appreciates the addition of the exclusion criteria for patients where the potential harms outweigh the benefits of treating a target HbA1c (e.g., patients in hospice, palliative care, SNP/long-term care, frailty, dementia, and a diagnosis of advanced illness). The measure encourages team-based care at the health plan level. ACP does not support this measure at the individual physician or group practice level since there is no testing data.
